How do I submit a claim?
You may submit your claim by mail or fax by filling out a claim form and attaching your receipts. You may also submit claims online by clicking the Flexible Spending Account Login link. You can then print out a claim form which you will need to send in to us with your receipts.

How do I access my account online?
Simply click the Flexible Spending Account Login link and then login with your username and password. If you do not have your username and password contact us at 888-266-1732 and we will be happy to provide you with this information.

What is my balance?
If you provide us with an email address on your claim form, we will email you your balance information with each reimbursement. You may also find your balance information at any time by logging into the website.

Do I need to activate my debit card?
No, there is no need to activate your debit card. It can be used immediately upon receipt.

How do I order a second debit card for my spouse?
Email us at clientservices@nextgenerationenrollment.com and list your name, your company’s name, your spouse’s name and his/her date of birth. You may also call us at 1-888-266-1732.

Where will I find out which stores are on the ‘approved vendors’ list for over-the-counter items?
For a list of all approved merchants who have, or plan to implement, the IIAS Standards required for card acceptance go to www.sig-is.org. On the left hand navigation bar, click the link titled “SIGIS Merchant List.”

My claim was denied, how do I find out why?

COBRA/HIPAA

How do I sign up for COBRA coverage?
Complete, in full, the COBRA election form and mail it back to Next Generation Enrollment. Our mailing address is PO Box 527, Ada, MI 49301. Your Qualifying Event notice will include the time period you have to elect COBRA Continuation Coverage. You do not have to make payment with your election. Payment must be made within forty-five days of when you postmark your COBRA election form. If you elect coverage, monthly remittance coupons will be mailed to you with payment information and the payment address.

When will my coverage be effective and will there be a lapse in coverage from when I originally lost coverage?
COBRA Continuation Coverage is not effective until your payment is received by Next Generation Enrollment. Once your payment is received, you will be reinstated into the plans you have elected to continue under COBRA Continuation Coverage. The reinstatement will be retroactive to the date in which coverage was originally cancelled so there will not be a lapse in coverage.

When is my payment due and is there a grace period?
Your initial COBRA Continuation Coverage premiums are due in full by postmarking your payment on or before forty-five days from the date in which you postmark your COBRA Continuation Coverage election form. Ongoing payments will be due on the first of each month. For example, for coverage from June 1 to June 30, your payment is due on June 1. However, as long as your COBRA Continuation Coverage payment is made in full and postmarked within thirty days of the due date, it will be accepted and your coverage will remain active. If your payment is not postmarked within thirty days of the due date, your COBRA Continuation Coverage will be cancelled.

Where do I send my payments?
Payments should be written to Next Generation Enrollment and mailed to PO Box 527, Ada, MI 49301.

Can I pay with a credit card?
Payment is only available via check or money order. Credit card payments will not be accepted.

Will I be getting new ID cards?
You will not be receiving new ID cards if you enroll into COBRA Continuation Coverage. Your coverage will be the same and your existing benefit cards will still work.

When can I change my coverage under COBRA?
At the annual open enrollment, you will have the opportunity to change your plan election. Your COBRA premiums may also change at the annual open enrollment.

How do I obtain a HIPAA Certificate of Creditable Coverage?
A HIPAA Certificate of Creditable Coverage was provided with your Qualifying Event notice and a duplicate can be obtained by contacting Next Generation Enrollment at cobra@nextgenerationenrollment.com or by calling 888-266-1732.
